Replacement Library Card Application

Form Instructions

This form is for cardholders who already have an account and are requesting a replacement card.

A lost card replacement fee will be added to your account. Fees are $2 for an adult replacement card and $1 for a youth card.

You will receive your physical Leon County library card in the mail within 10 days, but you can use your new library card number and PIN immediately.

Responsibility Statement

I am 16 years or older, or the parent/guardian of a youth 15 years or younger and hereby apply for the privilege to check out materials from the LeRoy Collins Leon County Public Library System. I agree to comply with all regulations, pay all fines, pay for the loss or damage of materials charged to this card, and give immediate notice of an address change or the loss of this card. A fee will be charged to replace a lost card. I understand that I am responsible for all materials checked out on this card and that my card and borrowing privileges are not transferable unless express written permission is given. Delinquent accounts may be handed over to a collection agency..
